The following questions need to be answered only if you are pleading guilty to one of the following first
or second crimes that occurred on or after June 29, 2001:
murder, aggravated manslaughter or manslaughter, vehicular homicide, aggravated assault, disarming a
law enforcement officer, kidnapping, aggravated sexual assault, sexual assault, robbery, carjacking,
aggravated arson by placing another person in danger of death or serious bodily injury, burglary, theft by
extortion by obtaining property of another by threatening to inflict bodily injury on, or physically
confine or restrain anyone or commit another offense, booby traps in manufacturing or distributing of
CDS facilities or strict liability for drug induced deaths.
